Implementation is another source of evidence. Monitor leading signals, counter-signals and lived effects; preserve the record; then update whichever part of the cycle the evidence challenges.

IIR is not a veto for incumbency.

Preservation receives the same scrutiny as intervention. A harmful or unlawful mechanism does not gain protection because it carries another useful function.

RIPPLE-4 is not a prediction score.

It exposes mechanisms, adaptations, couplings, critical tails and indicators. Uncertainty remains visible.

SOT does not rescue a failed critical gate.

It chooses and sequences among ready courses. It cannot trade away a blocked rights, safety or essential-continuity floor.

Proportional review

Require deeper review when failure would be costly or hard to reverse.

The level is chosen by credible downside, centrality, opacity and controllability. It is not a rank of political importance.

Low-downside, familiar and reversible change

02Standard

Material programme, rule, service or administrative redesign

Multi-source function map, distribution and rights screen, RIPPLE-4 map, successor coverage, triggers, strongest alternative and independent challenge.

03Full

Central, opaque, high-downside or hard-to-reverse intervention

Stress evidence, front-line and affected-group inquiry, rival causal accounts, full function-to-successor traceability, portfolio coupling, recovery plan and published minority view.

04Emergency

A genuine urgent threat with a compressed inquiry horizon

Minimum viable audit, immediate continuity bridge, narrow temporary authority, monitoring, incident log, sunset and mandatory reconsideration.

A 15-field public record for major policy decisions.

The record does not replace judgment. It makes the judgment inspectable, comparable and revisable.

Three vocabularies remain separateIIR reports each gate as met, conditional, open or blocked and recommends preserve, redesign, replace or remove. SOT then governs commitment through scale, modify, pause or stop triggers. None of these is a numerical ranking.
